  • Dysplasia of the elbow joint in the dog. Operative treatment PAUL (proximal abducting ulnar osteotomy) Vetriver Clinic.

    04/02/2021

The disease process that begins in elbow dysplasia results from uneven loading of the joint surfaces and overloading of the medial compartment of the joint. Surface conflict and compression lead to destruction of articular cartilage, loading and fragmentation of the coronoid process, with time leading to permanent degenerative changes.
The PAUL procedure involves performing an osteotomy of the ulnar bone. Then a special implant is fixed to change the way the forces in the joint work.
By distributing the forces in the joint evenly, we restore its function and prevent further destruction of the medial compartment along with the coronoid process.
The first procedure was performed in 2007 by Dr. Ingo Pheil in cooperation with Kyon, today we use this company's solution in the form of ALPS plate (advanced locking plate system)
The PAUL procedure should be preceded by a thorough diagnosis using arthoscopy or CT examination to determine the extent of the disease.

Merely changing the action of forces in the joint without verifying and removing damage that has already occurred, just as simply removing damaged structures from the joint without changing the congruence of forces acting in the joint, will not have a lasting effect.
